Practice information

Dr Hamilton delivers consultations exclusively via Telehealth only and is accepting a limited number of new patients by referral.

Secure Telehealth consultations are offered via Microsoft Teams, FaceTime or phone call in accordance with Medicare requirements. 

 

Our practice only treats Adult patients (17 years and older) and appointments must be arranged by the patient. A family member, partner or support person may also attend a consultation but please advise of their presence prior to the consultation. â€‹â€‹â€‹â€‹

Please note:

  • Our practice operates exclusively via Telehealth on a part-time basis, Monday to Wednesday only and is unable to provide crisis support.

  • Our practice does not does not accept referrals for medico-legal or court reports, or third party assessments such as insurance or work cover claims.

  • Our practice does not provide reports for NDIS, DVA or other third party providers.

  • Specific medical reports and letters sit outside of a consultation and attract a fee dependent on the complexity and length of time taken to complete. There is no Medicare rebate for this service. 

  • All prescriptions are provided via EScript only and any prescription renewals require a booked consultation. â€‹

Next steps to an Initial Appointment

  1. Please arrange a current and valid referral from your General Practitioner. This can be faxed or emailed through to our practice. 
     

  2. ​Reach out to us via email and introduce yourself. Please include your GP referral (if not already sent) and any other relevant medical reports from previous psychiatrists, psychologists or health professionals who have supported your mental health.
     

  3. Our friendly admin team will send you will a secure patient registration form to complete and will provide detailed information about the next steps, relevant fees and Medicare rebates. 
     

  4. Once your GP referral and completed registration form are received, you will be contacted and offered an appointment or placed on our waiting list dependent on our current caseload and capacity.
